If you request information about an annuity product represented on our website or ask for a personalized annuity quote, we collect certain personally identifiable information from you on our information request form. You must provide contact information (such as name, email, phone, and physical address) and financial information (such as the amount available for deposit and source of funds). If you apply for an annuity purchase, the issuing Insurance Company will require that you provide certain personally identifiable information, including your name, address, phone number, date of birth, and Social Security number. To further protect your privacy, My Annuity Store, Inc. always collects this information offline.

If you submit an annuity information request or subscribe to our newsletter, we may occasionally send you information regarding various annuity products, interest rate updates, and special offers. As is true of most websites, we gather certain information automatically and store it in log files.
